Clinician referral handoff
A clear next step from care to general fitness
Give your patient a private link to explore on their own, or introduce someone who has asked Sound Fitness to contact them. Both paths begin with a free assessment and keep clinical care with the patient's licensed providers.
Choose the handoff
Patient-led or clinician-introduced
No clinical records are needed. The difference is simply who makes the first contact.
Patient-led handoff
Let the patient choose the next step
Share the assessment link when you want the patient to contact Sound Fitness themselves. It asks about goals, schedule, location, and training fit before any appointment or package decision.
Suggested handoff
“You may be ready for general fitness support outside the clinic. Sound Fitness offers in-home coaching and starts with a free assessment so you can decide whether it feels like a good fit.”
Appropriate scope
- The patient is ready for general exercise or has the clearance their provider requires.
- The goal is ongoing strength, balance, mobility, or exercise consistency—not diagnosis or treatment.
- Training can happen safely in the patient's home or available fitness space.
